General Instructions.

These are all Microsoft Excel document downloads.

Our preferred submission form is with Microsoft Excel but almost any spreadsheet program will work. An ASCHI file with delimited columns (ie. columns are separated by either one space, one tab, or a comma) would also be acceptable. The columns must be in the correct order (reading from left to right).

If you have problems downloading or opening your file let us know and we will fax you a copy of the spreadsheet. You will need to recreate the headers to correspond with the original Excel file.

Depending on your browser preferences and whether or not you have Excel installed on your computer, you will be asked if you wish to "open the file" or "save the file." We recommend you "save the file" and then open it using the Microsoft Excel software or the spreadsheet program of your choice (which may automatically convert it to a usable format).
If you do not have Excel installed on your computer, Microsoft has a File Viewer available for download from here. If you already have Excel installed DO NOT install the Excel File Viewer in the same directory. Certain critical files will get overwritten and render Microsoft Excel unusable.




Anthropology Component

The Anthropology Demographic and Background Data Entry Form has two sections used to enter Background data for Anthropology Component Populations. The data entered in these two sections, the Population Background Data (blue), and the Subject Background Data (green), are required.

Additional Family Background data on the subject's ancestors may also be entered (optional). Enter these data as separate entries for each individual in the Subject Background Data (green) section.

The Read Me document explains the fields listed in this form and describes the structure and use of the unique ID codes referred to in the two sections.



Laboratories participating in the Anthropology Component can now submit "available" data generated using alternative (non-13th IHWG) typing methods for analysis by the Biostatistics Core and subsequent inclusion in the central database. The database will be able to accommodate data generated as part of the 13th IHW (using IHWG reagents), as well as other available data generated using non-IHWG methods.

Toward this end, Anthropology Component participants are encouraged to download the data template (for alternative typing methods) for their use. The data template is an excel file that presents all of the required datafields in an organized, easy-to-fill-in fashion. The first worksheet of this excel file documents the required data format for this template. In addition, a sample data file (see below) provides an example of a properly filled-in data template.

Cancer Component

Cancer Component data entry forms are available for various diagnosis in order to capture the disease specific data.

Download and save the excel file to a local drive before entering any data. When you are ready to submit your data, save your file using your lab directors last name (example, cancer_hansen2.xls, cancer_hansen3.xls, etc.) and send as an email attachment to: database@ihwg.org.




Human MHC Component

Download and save the Microsoft Excel file Human MHC entry form to a local drive before entering any data. The excel file includes instructions (as clickable text boxes) for filling out the demographics section of the form and tables that give lists of allowable data entries. Completed forms should be sent as e-mail attachments to database@ihwg.org. For questions, please contact Gary Schoch at gschoch@fhcrc.org.




Cytokine Polymorphisms

If you use the Cytokine-enhanced SCORE (will work only with the "Heidelberg PCR-SSP Cytokine Set"): Enter all reaction patterns in SCORE and Export all data you would like to share by using the "Export data" Option in Score (For transfer use the file: "transferLABCODEDATE.vda", which is located in your Score root-directory. Select the samples you want to transfer. This will generate a file with the name: cytokineLABCODEDATE.xls. This file contains all information, which is required for the evaluation of the cytokine data.

If you don't use SCORE: Download and save one of the subsequent excel files to a local drive before entering any data. To submit cytokine gene polymorphism data for samples tested with the SSP assay or other methods, please use the Phase I or Phase II Data Capture Forms.

Hematopoietic Stem Cell Transplantation

Participants in the Hematopoietic Cell Transplantation Component can now download the database submission form. This is a Microsoft Excel file template. You will also find brief instructions explaining navigating an Excel document, and filling in the cells of this template.

Chronic Rejection Registry
